Purchase and Replacement Guarantee
Under the terms of this agreement, the
undersigned parties consent to the following conditions in regard to the sale of the
GOLDEN Retriever puppy identified below:

The BREEDER of the above puppy, SUSAN
KIEFER, agrees as follow:
That this puppy will be REPLACED by another of the same sex from a future CHOCTAW
litter if:
- Your puppy is diagnosed with HIP or ELBOW DYSPLASIA.
a. X-rays must be taken before
26 months of age.
b. Proof must be sent to me (OFA letter).
- Your puppy is diagnosed to have Juvenile Cataracts, Entropion, or Ectropion.
a. Puppy
must be diagnosed by a LICENSED DIPLOMATE of the AMERICAN COLLEGE OF VETERINARY
OPTHAMOLOGISTS.
b. Puppy must be diagnosed by the age of 26 months.
c. Proof must be sent to me (copy of the OPTHAMOLOGISTS REPORT).
- Your puppy is diagnosed to have SAS (Subaortic Stenosis).
a. Puppy must be diagnosed by a Licensed Board Certified Cariologist.
b. Puppy must be diagnosed by 26 months.
c. Proof must be sent to me (Copy of Cardiologist Report).
That this puppy will be
REPLACED when: confirmation of Spay/Neuter.
This contract is NULL and VOID if this puppy is bred or breeds another or is sold,
given away or is traded for another. This contract will be NULL and VOID if my
kennel name of CHOCTAW is not used as a prefix when registering your puppy.
The purchaser(s) of the above identified puppy, agrees as follow:
That the purchase price for the puppy shall be $________ (M), $_________(F).
A deposit of $200 will hold this puppy. This deposit
can only be refunded when anotherperson has purchased
the puppy. The balance of $________ (M),
$_______(F) plus the cost of the shipping crate $30), airport fee ($50) is due BEFORE the
puppy is released. Any replacement puppy will be subject to the same terms outlined
in this agreement and guarantee.
IF FOR WHATEVER REASON YOU CANNOT KEEP YOUR PUPPY, I MUST BE CONTACTED. I HAVE
THE OPTION TO EITHER BUY THE PUPPY BACK OR DECLINE.
After reading the above terms and conditions, WE find them mutually agreeable and
acceptable.
